essential.


The Palliative Care Nurse Practitioner provides consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician.

RESPOSIBILITIES:
Facilitates clarification of patient and family goals of care.
Facilitates access to appropriate support models of care i.e. hospice, rehabilitation and on-going aggressive treatment.
Assists with diagnosis, treatment and management of acute and chronic health conditions.
Consults with Hospice and Palliative medical director as needed, informs primary physician of recommended services and collaborates with other physicians as needed.
Educates facility staff, co-workers and the community on all aspects of palliative care and the services provided by Palliative Medical Associates.
Attends and participates in interdisciplinary Palliative Care team meetings.
Consults with the referring physician, the acute care team and other involved providers to contribute to the patients plan of care.
Prepares and maintains accurate patient records, charts and documents to support the medical practice and reimbursement for services provided.
Prescribes medications including controlled substances to the extent delegated and licensed after collaboration with palliative care and referring physician.
Orders diagnostics / treatments as needed after collaboration with palliative care and referring physicians.
Assists in development of clinical practice guidelines / standards in support of quality palliative care as requested by supervisor.
Conducts face to face visits on current Hospice patients if needed

Requirements:
REQUIREMENTS:

Certified BLS Provider specializing in Health Service Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date. Or military CPR-First Aid training
Current DEA license in good standing
Licensed Nurse Practitioner credentialed from the New York State Board of Nursing or current home state license for multi-state license recognition Compact State obtained prior to hire date
Licensed Registered Nurse credentialed from the New York State Board of Nursing or current home state license for multi-state license recognition Compact State obtained prior to hire date
Master of Science from an accredited program
Minimum of five (5) years nursing experience.
Clinical nursing care experience in one or more of the following; palliative care, hospice, oncology, geriatrics, intensive care.
Current ACHPN certification or obtained within 1 year of hire
Two years of NP palliative care experience (preferred)
Reliable transportation, current state drivers license and automobile insurance.
